Safety That Lasts

A Playbook for Leaders Who Are Done with Compliance Theatre

Serious injuries and fatalities have hit a plateau. More rules will not fix this problem.

Decades of investment in compliance systems reduced minor incidents. But these systems have stalled. The accidents that still kill people are not caused by careless workers. They are caused by systems that strip away defences faster than a procedure can rebuild them.

Every hour spent managing a preventable incident is an hour lost from growth. A single serious event can erase years of profit. This book provides an intervention. It replaces the compliance playbook with a framework for managing how work actually happens.

What is inside

  • The financial architecture that traps middle managers, and how to dismantle it.
  • Leadership habits of organisations that sustain extraordinary safety records.
  • A 12-month transformation playbook with concrete actions.
  • The truth about why your safety dashboard is lying to you.

The Framework

The architecture relies on three main pillars:

  • Rethink Risk Management: Target low-frequency, high-severity risks. Build layered controls that assume human error. Verify controls on the shop floor.
  • Rebuild Management Systems: Strip away safety clutter. Create visual procedures that match reality. Build rules with the people who execute the tasks.
  • Grow Safety Culture: Treat safety as a non-negotiable business reality. Shift investigations away from finding a culprit. Develop teams with practical judgement.
